Family Doc's Clinic is dedicated to providing you with the highest quality, cost effective medical care. We accept all major credit cards and offer payment installments.

We accept most private insurance plans including:

  • Blue Cross and Blue Shield

  • Blue Cross and Blue Shield PPO

  • PPO Plans

  • Medicare

  • Aetna

  • Cofinity

  • Multiplan

  • Priority Health

  • We do participate in some HMO plans.

Due to the ever changing health insurance plans, please call our office to find out if we participate in your specific plan. You should also contact your insurance provider to confirm.

  • Pain Manangement Agreement

    • If you are a patient that requires controlled pain medication, you must sign a Contolled Substance Contract. Speak with your doctor about this or download the form here and bring it to your next appointment.
